Regarding appointment requests in our new Cumberland office:

I am currently accepting new appointments for my nutrition coaching program in our Cumberland office at 2222 Diamond Hill Road. Due to current scheduling demands I can not guarantee your first request for an appointment. Please leave several options if you have specific times needed and I will do my best to meet your scheduling preferences. Once you submit this form to my office I will contact you to confirm your request and to verify that the time requested is available.

Phone inquiries are also welcome by calling my office (401) 632-0868. Due to the volume of calls I receive for appointment requests I prefer to schedule your appointment online to best respond to your initial requests. If you provide your phone number, I will always call you to speak with you in person to confirm your appointment prior to our meeting.
