I will do my best to meet your scheduling preferences. Once you submit this form to my office I will contact you to confirm your request and to verify that the time requested is available.

Phone inquiries are also welcome by calling my office (401) 632-0868. Due to the volume of calls I receive for appointment requests I prefer to schedule your appointment online to best respond to your initial requests. If you provide your phone number, I will always call you to speak with you in person to confirm your appointment prior to our meeting.
